Since 2014 Face To Face Medical Management has been providing physician services to any and all patients who cannot get to an office. We provide transitional home care services by utilizing innovative technology to deliver prompt and convenient healthcare. Our board certified physicians, physician assistants, and nurse practitioners come directly to your home through a Face to Face visit or a Tele-health visit. We collaborate with other healthcare providers including hospitalists, primary care physicians, specialists, and home healthcare agencies.
