Our team consists of physicians, physicians' assistants, nurse practitioners, and wound care specialists. Our Board Certified physicians, physicians' assistants, and nurse practitioners come directly to you through an in-home visit or a Tele-health visit to deliver prompt and convenient healthcare.

At Face To Face Medical Management we provide physician services to any and all patients who cannot physically get to an office. We provide transitional home healthcare services to patients with complex medical needs. Our goal is to successfully transition homebound patients back to their primary care providers by providing a customized healthcare program based on the patient's needs.

We partner with medical professionals in the community to coordinate immediate care resulting in less emergency room visits and hospital re-admissions. We work closely with home healthcare agencies, PCA companies, medical social workers, registered nurses, licensed practical nurses, physical and occupational therapists, and dietitians.

If you are finding services for a new patient, please have the patient's name and insurance information available.
